Transition of Care

During this transition period, February 1 through March 1, 2012, any authorizations previously issued by Medicaid or Medicaid Providers (examples are surgeries, procedures, equipment, or other) will be honored by Louisiana Healthcare Connections for 30 days, and more information can be obtained by calling our toll-free number, 866-595-8133.

Louisiana Healthcare Connections (LHC) can be reached by calling our toll-free number, 866-595-8133 (for TDD/TTY services dial 877-285-4514). Our local staff is available to answer member and provider questions starting February 1, 2012 Monday – Friday from 7:00 a.m. to 7:00 p.m. Central Time. If you are not sure which department you need to reach, our Member and Provider Services department will speak with you to answer your questions, or refer you to the appropriate department that can assist you. During this transition, rest assured that our Integrated Care Team (ICT) will work to ensure the continuity of care and coordination of services for our new members entering the plan, and for those receiving active services who leave the plan. At Louisiana Healthcare Connections, we focus on coordination of care. This means that our staff will work with our members and providers to help bring together the medical, social, and financial services needed by the member, and ensure appropriate referrals and connections are made for the member to the appropriate providers or community resources. Our Medical Management department employs a dedicated Transition Coordinator that will work closely with the DHH Medicaid Managed Care Program staff and staff from other BAYOU HEALTH plans to ensure a safe and orderly transition for the members, especially those with special health care needs.
