- Online: Log into your account or create one at MyPlan.Healthy.LA.gov/en/my-account.
- Mobile app: Download the Healthy Louisiana mobile app for free.
- Phone: Call Healthy Louisiana at 1-855-229-6848 (TTY: 1-855-526-3346).
- Fax: Send the enrollment form to 1-888-858-3875.
- Mail: Send the enrollment form to PO Box 1097, Atlanta, GA 30301-9913.
Yes. Everyone on Medicaid can visit MyPlan.Healthy.LA.gov/en/my-account to change their health plan.
If you didn’t receive a letter, it probably means Medicaid doesn’t have your up-to-date address or email address. You can update your contact info at MyPlan.Healthy.LA.gov/en/my-account or by calling Louisiana Healthcare Connections at 1-866-595-8133 (TTY 711), Monday through Friday, 7 a.m. to 7 p.m.
You can go to MyPlan.Healthy.LA.gov/en/my-account to check and choose your health plan.
- If you change to another health plan after 6 p.m. on February 27, 2023 through 6 p.m. March 30, 2023; your new plan will start on April 1, 2023.
- If you change to another health plan after 6 p.m. on March 30, 2023 through 6 p.m. March 31, 2023; your new plan will start on May 1, 2023.
No. You do not need a reason to change your health plan at this time. Anyone receiving Medicaid can change their health plan at MyPlan.Healthy.LA.gov/en/my-account. After March 31, 2023, you can only change your health plan with a reason approved by the Louisiana Department of Health.
Yes! As an extra benefit, our adult members get up $500 in dental services each year starting January 1, 2023.
(If you saw a comparison chart that said we do not offer an adult dental benefit, it is not correct. Louisiana Healthcare Connections absolutely is offering this adult dental benefit in 2023.)
This change may affect your choice in doctors and specialists. Having access to the doctors you need is important for your health. If you choose a health plan with a small provider network, there will be fewer doctors you can see.
Louisiana Healthcare Connections has the largest provider network of any Medicaid health plan. This gives you access to the most specialists, mental health providers, primary care providers, vision providers, obstetricians, and more.
If there’s a certain provider that’s important to you, you can use our Find a Provider tool to make sure they are in our network.
This will not affect your Medicaid coverage. However, if you do not choose Louisiana Healthcare Connections, you will miss out on our extra benefits and rewards.
New Louisiana Healthcare Connections members will get a welcome packet and ID cards in January.
If you are currently a member and stay a member, you will not receive a new ID card unless you request one. If you are a current member and need a new ID card, you can request one through the member portal or by calling us at 1-866-595-8133 (TTY: 711) Monday – Friday, 7 a.m. to 7 p.m.
