We can help provide support, talk to your providers/doctors and connect you with specialists to help you or your child.

Choosing a PCP

When you become a Louisiana Healthcare Connections member, you must choose a Primary Care Provider (PCP) or we will assign you one. Your PCP will be your main doctor. You should go to the same PCP to get medical care on a regular basis. Your PCP is your “medical home.” This helps ensure you get the healthcare you deserve and stay in good health. You have the freedom to choose any PCP in our network. Your PCP can be the following types of providers:
  • Pediatrician
  • Family General Practitioner
  • Internist
  • Obstetrician/Gynecologist
  • Specialist who performs PCP functions
  • Nurse Practitioner

Women can also choose a women’s healthcare provider in addition to a PCP. Please refer to the Member Handbook for more information about choosing a PCP and getting healthcare. If you do not select a PCP, Louisiana Healthcare Connections will assign one to you. The assignment will be based on:

  • Claims history
  • If a PCP has seen you before
  • Family history with a PCP
  • How close you live to an available PCP
  • A PCP type that is right for you based on your needs

If you would like to change your PCP, Louisiana Healthcare Connections can help. Simply fill out and return the PCP Change Request Form. You can also log into the member secure portal, or call Members Services at 1-866-595-8133.
