On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions. Take healthy steps to earn My Health Pays® rewards.

Yearly Health Screenings

  • $50 - Breast Cancer Screening
    Ages 50-74. One per year. 
  • $50 - Cervical Cancer Screening
    Ages 21-64. One per year. 
  • $50 - Colorectal Cancer Screening
    Ages 45-75. One per year. 
  • $50 - Diabetes Care Screenings
    For members with diabetes, ages 18-75. Must complete all of the following once in the calendar year: HbA1c test, kidney screening, and retinopathy screening (dilated eye exam).
  • $25 - Sexually Transmitted Disease Screenings
    Must complete all of the following once in the calendar year: HIV 1 or 2 test, chlamydia screening, and a gonorrhea screening.

Care for Pregnancy and New Moms

  • $30 - Complete Notification of Pregnancy (NOP) Form
    Log in and complete online or call Member Services at 1-866-595-8133, Monday – Friday, 7 a.m. to 7 p.m.
  • $110 - Prenatal Visits
    $10 for each prenatal visit, up to 11 visits. Must complete NOP first.
  • $50 - Postpartum Visit
    Between 7 and 84 days (1-12 weeks) after delivery. Must complete NOP first.

Helping Your Child Stay Healthy

  • $30 - Infant Well Visits
    For completing six infant well visits by the 15th month birthday. At 2, 4, 6, 9, 12 and 15 months.
  • $50 - Vaccinations for Adolescents
    For receiving the following vaccinations before the 13th birthday: one dose of meningococcal vaccine, one dose of Tdap vaccine, and two doses of human papillomavirus (HPV) vaccine.

Making Healthy Changes

  • $25 - Trying a Medication or Counseling to Help You Quit Smoking
    For counseling or filling a prescription for a covered smoking cessation drug or product. One per year.

Your My Health Pays® reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays® Visa® Prepaid Card will be mailed to you.

This My Health Pays Rewards Visa Prepaid card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC. Card cannot be used everywhere Visa debit cards are accepted.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact Member Services at 1-866-595-8133.

Once you receive your My Health Pays® rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
