Louisiana Healthcare Connections Announces Grant Winners

BATON ROUGE, La. – Louisiana Healthcare Connections has awarded six community organizations a total of $12,000 in grant funding to support local projects that address social drivers of health, expand access to preventive care, and improve health outcomes for Louisiana residents.
“Even small investments in community-led programs can create powerful change,” said Yolanda Wilson, Senior Vice President of Quality Improvement at Louisiana Healthcare Connections. “By funding innovative local initiatives, we are helping remove barriers to care and building healthier communities across our state.”
Grant recipients were selected for their commitment to addressing health-harming social needs through community-centered solutions:
The Walls Project will expand its farm-based wellness programs to reach more families in neighborhoods disproportionately impacted by health disparities.
Team Capables will strengthen services for individuals with disabilities, supporting Autism Walks, youth sports programs, and educational workshops that promote physical activity.
WeHelp NOLA will broaden outreach, providing no-cost mental healthcare and wellness services to hospitality industry workers in New Orleans.
The Highland Center will expand two of its core community meal programs, the Blessing Dinner and the Senior Lunch, working to reduce food insecurity in northwest Louisiana.
Hellfighters West Monroe, LA Unit will broaden its outreach to vulnerable residents in northeast Louisiana by expanding its food pantry and increasing the number of meals provided to the homeless.
Gifted Smiles will expand access to dental education and preventive care, promoting better oral health as a foundation for overall wellness.
“With multiple grant cycles taking place throughout the year, Louisiana Healthcare Connections will invest $200,000 in local initiatives that align with our mission of care,” said Jamie Schlottman, Plan President. “Together, we are building a stronger, healthier Louisiana.”
