No matter what stage of life you are in, Sharecare can help you take your health to the next level! Plus, when you take your RealAge Test and/or complete Craving to Quit (our tobacco cessation program), you earn rewards.

- Visit LaHealthConnect.Sharecare.com and follow the steps to create your account.
- Take your RealAge Test by selecting You and Your RealAge. When you complete it, you’ll earn a $10 reward! Your RealAge is how old you really are, based on your lifestyle, health history, and more. Once you know your RealAge, you’ll get recommendations on how to lower it over time and become a healthier you.
- Download the Sharecare app onto your mobile device to keep track of your health and complete activities on-the-go.
Join fun wellness challenges.
Sharecare’s wellness challenges are designed to motivate you to improve your health. You can use your Sharecare health trackers to complete challenge goals and earn bragging rights. Join us for some friendly competition this year!

RealAge Test Reward: When you register for an account and complete your RealAge Test you’ll earn a $10 reward.
Craving to Quit Reward: Members who use tobacco products and want to quit can complete the Craving to Quit program and earn a $10 reward.

You can check your results by visiting your health profile at LaHealthConnect.Sharecare.com. Then select You and Your RealAge. If you are unable to complete the whole RealAge Test at one time, you can pause the test. When you do, your answers will be saved and you can complete the test later. It only takes 10-15 minutes to complete!
No. As a Louisiana Healthcare Connections member, his program is available at no cost to you. In fact, members who complete the RealAge Test and/or the Craving to Quit program can earn rewards!
Yes. Your privacy is ensured in compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996. Sharecare adheres to strict security guidelines. None of your personal information will be shared without your permission.
The trackers you should update daily will be based on the results of your RealAge Test. If you join a specific challenge, you should regularly update the tracker for that challenge to meet the goal.
Craving to Quit is a 21-day smoking and vaping cessation program that helps you quit tobacco for good. The program teaches you how to retrain your brain by showing you how habits are formed and how to break them.
Log into your Sharecare account at LaHealthConnect.Sharecare.com. Then select Achieve, Programs and Craving to Quit. You’ll be asked to take a quick assessment to see if you qualify for this program. If you are eligible, select Get Started. You must complete all modules of the Craving to Quit program by December 31, 2023 to earn your $10 reward!
