2017-15: Final Reminder: HEDIS Hybrid Season Records Request

HEDIS Hybrid Season is an important part of Louisiana Healthcare Connections’ efforts to accurately assess the health of our members and communities. As part of our efforts to conduct an efficient, accurate community health assessment, we have faxed to you a Medical Record Request and Member Pull Lists.

As a friendly reminder, please send requested medical records to Louisiana Healthcare Connections via fax at 1-877-285-3966.

If your office did not receive a fax request with an attached member pull list, please contact the HEDIS Department at 1-225-201-8562.

If you prefer, Louisiana Healthcare Connections also has a retrieval team available to pick up records onsite. To schedule this service, please contact Mary Guilbeau at 1-225-663-5774.

These record requests are a required component of the Provider/Group/Hospital Contract Agreement, Article III, Section 3.11, of the Coordinated Care Network – Prepaid Program Requirements, which states, “Provider shall submit all reports and clinical information required by the CCN, including but not limited to, HEDIS, the Agency for Healthcare Research and Quality (AHRQ), and EPSDT.”
