2018-44: Change in Behavioral Health Utilization Management Fax Numbers

Effective September 17, 2018, the Behavioral Health Utilization Management fax numbers for both inpatient and outpatient authorization requests will change. For the next 30 days, both the old and the new fax numbers will be in operation, so you can begin using the new numbers immediately. After September 17, 2018, the new fax numbers will be required. Please update any saved contact records accordingly.

The fax numbers on our behavioral health authorization request forms have been updated to reflect this change. If your agency has saved previous versions of the forms, we encourage you to download updated forms.

For your reference, below are both the old and new fax numbers for authorization requests:

Outpatient

• Old: 1-844-466-1277
• New: 1-888-725-0101

Inpatient

• Old: 1-866-535-6974
• New: 1-866-698-6341

Additionally, we encourage behavioral health inpatient and residential providers to submit requests via fax, rather by phone, for more convenient and efficient reviews.
