Louisiana Healthcare Connections is sharing a revision made to 340B Drug Pricing Program (IB25-27) on December 2, 2025. 

Revisions include: The change from "JG to "TB" under the Medical Claims Submitted by a 340B Covered Entity the use of modifier codes listing.
