Additional Third Party Liability Carrier Code Updates (IB 25-16)

Louisiana Healthcare Connections is sharing Additional Third Party Liability Carrier Code Updates (IB 25-16) made in an effort to streamline TPL identification and claims adjudication process.
The Louisiana Department of Health will continue to consolidate carrier names and codes where duplicates exist on the current LDH Carrier Code listing. Deactivating duplicate carrier listings when multiple records exist for the same insurance carrier allows providers to identify the proper carrier code to report new or updated TPL as well as submit claims without having to report multiple carrier codes on the claims submission.
Effective January 1, 2024, the Medicare Advantage carrier previously known as Peoples Health moved to the UnitedHealthcare (UHC) network. All providers are expected to use the UnitedHealthcare tools and processes, including eligibility, benefits, claims and authorizations for dates of service January 1, 2024 forward. To align with the merging of Peoples Health to UHC, the carrier name UHC Medicare Advantage (H39210) will change to UHC Medicare Advantage, including Peoples Health (H39210). For all non-Medicare Advantage UHC plans, providers should continue to utilize United Healthcare (433100). Additionally, the below carrier codes will be retired and providers should utilize only H39210 or 433100 moving forward.

The following codes for United Healthcare/Peoples Health are retiring:
- 185100 (UnitedHealthcare)
- 433000 (UnitedHealthcare)
- H50260 (Peoples Health, A UnitedHealthcare Company)
As part of this project, LDH is also consolidating the following duplicate carrier records:
Aetna
- Aetna Life and Casualty (082800) will be renamed to Aetna and will be the default carrier moving forward.
- Aetna (105000) will be deactivated.
WellCare:
- WellCare (H19030) will be the default carrier moving forward.
- WellCare of Louisiana (328300) will be deactivated.
The above consolidations began April 21, 2025 and were completed by May 1, 2025.
This list will be updated continuously and the carrier code listing will be updated at LaMedicaid.com.
