Important Reminder About Your Medicaid Eligibility

You may have received a phone call or letter from Louisiana Medicaid asking you to renew your coverage, provide more information, or both.

If you need to renew your coverage:
The best way to renew is to do it online through the Self-Service Portal at www.Healthy.LA.gov. The portal is available on any computer or mobile device. On the portal, you can fill out an application, renew your coverage, or make changes to your information.

If you need to submit information:
Look in your letter for the section with the heading that says, “Information We Need From You.” This section will tell you what information is needed. It will also tell you the date the information is due. You can submit this information by email, fax or mail, or through the Medicaid Self-Service Portal:

Remember: It’s important to respond to letters from Louisiana Medicaid.

If you don’t renew your Medicaid or provide the information requested, your case may be closed and you could lose your Medicaid coverage.
