Share your opinions about health in your community!

Help LDH better understand what impacts health the most in your community. By taking this 10-minute survey, you will be able to share your opinions on the health issues you think are most important to address, what could make your community healthier, and more. The survey is anonymous and will be open until September 10, 2021. The survey is available in English, Spanish, and Vietnamese.

The survey is part of the Louisiana Department of Health’s State Health Assessment, which occurs every five years to address population health and health barriers that prevent Louisianans from being as healthy as they can be. Information from this survey will be used to inform the State Health Improvement Plan.

Achieving our goal of healthy people and healthy communities is a complex task that requires participation from as many people and organizations as possible, to ensure all Louisianans are represented. Knowing this, we rely on feedback from people in communities throughout Louisiana, as well as external stakeholders and partners.
