Louisiana Healthcare Connections Announces New “Transforming Health Award”

Louisiana Healthcare Connections has introduced a newly named physician recognition, the Transforming Health Award, honoring healthcare providers who not only achieve top performance in Healthcare Effectiveness Data and Information Set (HEDIS) quality measures, but are also making meaningful, measurable differences in the health of their communities.
The inaugural recipients of the Transforming Health Award include three physicians/physician groups representing key areas of care across Louisiana:
- Dr. Samuel. Brown, Kenner
- New Orleans East Louisiana (NOELA) Community Health Center, New Orleans
- Lily Pad Pediatrics, Opelousas
These three were selected for their outstanding performance in critical quality metrics, including preventive care, maternal health, and chronic condition management, paired with a demonstrated commitment to improving access, outcomes, and patient engagement in their respective communities.

Each honoree represents a critical component of Louisiana’s healthcare ecosystem, from pediatric care to community-based primary and maternal care, areas that are central to improving long-term health outcomes across the state.
