Healthy Rewards

Vision Care

Yearly eye exams for the whole family are covered. If needed, children under age 21 get up to three pairs of glasses. Adults (21+) may choose glasses or a 1-year supply of contact lenses. Find an in-network eye doctor near you.  

Hearing Care for Adults (21+)

One hearing exam per year, plus hearing aids every two years if needed. And hearing aid batteries are covered too. (Children’s hearing care is covered by Medicaid.) Have concerns about your hearing? Talk with your Primary Care Provider (PCP) to schedule a test.

Sharecare Health & Wellness

Download for free, then take the RealAge Test and earn $10. If you use tobacco or vape, quit for good and earn $10 more with the Sharecare Craving to Quit® program.

Health Coaching and Support

Members in care management may qualify for extra services like:

  • Health coaching for weight loss and diabetes
  • Alternative treatments for chronic pain management
  • Meal delivery after hospitalization for certain conditions
  • Home services to improve health-harming issues, like mold cleanup and pest control
  • Transportation to food pantries, pharmacies, WIC appointments and more
