New for Your Convenience: QR Code on Member ID Cards

Beginning in December, Louisiana Healthcare Connections member ID cards will feature a QR code of their member ID number. This new feature can speed up check-in and administrative processes at provider offices that use 2D barcode scanners. 

Over the next six months, there will be multiple member ID card formats in use while we re-issue member ID cards. Examples of these formats are included below, and all are valid ID cards. By June 2021, all member ID cards without QR codes will be replaced by ones that include the QR code.

Reminder: Eligibility changes can occur at any time and possession of a member ID card is not a guarantee of current eligibility. Always verify member eligibility online or by phone on the date of service.
