Provider Notice: Remote Patient Monitoring Available September 1, 2024

Starting September 1, 2024, Louisiana Healthcare Connections is thrilled to announce a new partnership with Ochsner Health Services. Through this collaboration, Ochsner Digital Medicine's Remote Patient Monitoring (RPM) program will be available to help members manage high blood pressure and Type 2 diabetes. This service provides one-on-one clinical support and monitoring, allowing members to receive care from anywhere.

Louisiana Healthcare Connections will identify members who may benefit from the RPM program, and Ochsner Digital Medicine will reach out to these members to discuss potential enrollment. Additionally, providers can request the enrollment of their patients into the program.

This initiative aims to enhance patient care by offering a convenient and efficient alternative to in-person services.

Providers are invited to attend a informational and educational presentation and Q&A opportunity on August 23, 2024, from 2:00-3:00pm.
