Renewing Your Medicaid Coverage Just Got Easier

If you get a letter from Louisiana Medicaid letting you know it’s time to renew your coverage, there’s good news. The Louisiana Department of Health (LDH) is making the process easier by including pre-filled forms in renewal letters.

When it’s time for you to renew, you’ll get a paper renewal form in the mail already pre-filled with your information. First, check that the information is correct. If you need to make changes, just write them in the spaces provided. You can add additional people in your household or list another job. Then, sign the form and return it.

You’re already connected to the care you need, so don’t risk losing it! Even if all the information on your pre-filled form is correct, you will still need to sign and return it to complete the renewal process. You must complete your renewal process by the due date on your letter. If you miss the deadline, you will lose coverage and must reapply for Medicaid.

You can renew online and by phone. You can also return your form by fax, email or in person.

Be sure your mailing address is current so you’ll get your letter when it’s time to renew! You can make changes to your address online using the Louisiana Medicaid self-service portal. Or call the Louisiana Medicaid hotline at 1-888-342-6207, Monday through Friday, from 8 a.m. - 4:30 p.m.
