Provider Notice 20-15: Vitamin D Testing Policy

In conjunction with the Louisiana Department of Health (LDH), Louisiana Healthcare Connections has developed a joint, pilot program to cover the screening of Vitamin D levels in members with certain diagnoses. If, after initial screening, a member is determined to be Vitamin D deficient, testing can be conducted.
Effective December 1, 2020, the following conditions must be met for reimbursement of Vitamin D Testing:
- Members must have a diagnosis where screening is considered medically necessary, as determined by the Centers for Medicare and Medicaid Services (CMS) and cited later in this policy.
- When a member meets the above criteria, the limit for Vitamin D screenings will be no more than one (1) per year.
- When a member is diagnosed with Vitamin D Deficiency, Louisiana Healthcare Connections will cover a maximum of four (4) tests per year.
- The diagnosis, along with the appropriate CPT Codes, must be submitted on the claim for the screening.
