2024-04: New Standardized Forms for BH Inpatient and Outpatient Authorization Request

Louisiana Healthcare Connections is committed to facilitating prompt medical necessity reviews for the benefit of our members' high-quality outcomes. Providers can submit service requests conveniently through our Secure Provider Portal (provider.louisianahealthconnect.com) or by fax. In our ongoing efforts to enhance the efficiency of the authorization process, we have transitioned to using standardized fax documents for authorization requests.

For Inpatient Authorization requests, providers can use one designated form and submit it via fax at 866-698-6341. Similarly, a separate standardized form is designated for Outpatient Authorizations, covering both Behavioral Health authorizations, and can be submitted via fax at 888-725-0101. This streamlined approach aims to simplify the authorization process and ensure a more efficient experience for both providers and members.

By implementing standardized forms, we streamline treatment request procedures, consolidating multiple forms and providing providers with increased flexibility in presenting their clinical information.
