Healthcare Provider Webinar Introduces New Adolescent Toolkit

Louisiana Department of Health Featured in Upcoming Louisiana Healthcare Connections Lunch & Learn
Baton Rouge, LA. – Louisiana Healthcare Connections, in partnership with the Louisiana Department of Health (LDH) and the Louisiana Chapter of the American Academy of Pediatrics (LA AAP), invites healthcare professionals to attend a free webinar unveiling the newly released Adolescent Well-Care/Youth Health Care Transition Toolkit.
This virtual event will take place on Tuesday, June 17, 2025, at 12:00 p.m. CST, and is free to attend with advance registration. Interested healthcare professionals should sign up online.
The toolkit was developed to support improved adolescent health outcomes and smooth transitions from pediatric to adult care. Designed for use by healthcare providers, it includes:
- Quality improvement worksheets
- Referral guides
- Best practices for supporting youth through critical health transitions
Featured Speaker: Patti Barovechio DNP, MN, BSN, CCM, Statewide Nurse Consultant, Bureau of Family Health, Louisiana Department of Health
Dr. Barovechio brings over 40 years of nursing experience spanning neonatal to geriatric care, with a current focus on systems-level public health strategies. Her current work focuses on strengthening pediatric primary care systems and ensuring that all Louisiana youth have access to high-quality, holistic healthcare. Her doctoral project directly contributed to the creation of a statewide youth transition resource SmoothMovesYHT (Youth Health Transition).
