2021-18: New MHR Form Beginning June 30, 2021

Louisiana Healthcare Connections is replacing the initial and renewal 60 day Outpatient Treatment Request (OTR) forms with a single OTR form to streamline the authorization process. The new OTR form will give Licensed Mental Health Professionals (LMHP) the opportunity to provide clinical documentation to support the reason for initial and continued services.

MHR and PSH providers requesting CPST and PSR services must use the new form beginning on June 30, 2021.

A free on demand training has been developed to assist providers with the new form and highlights of improvements.

If an old form is submitted after June 30, 2021, it will be send back to the requesting provider with the ask for the provider to use the new form.
