How can we serve you better?

As a Louisiana Healthcare Connections member, we want to know what you think about your health plan and our services.

You may have received a letter recently asking you to complete a survey about your health plan. Your input matters to us. Your answers will help us improve our member experience.

Because we are asking only a few people to take the survey, it is very important that you complete the survey right away. It will take you less than 20 minutes to complete. You can complete the survey electronically or by mail.

Remember: The survey only helps if everyone who gets the survey sends it back, so please complete it as soon as possible.

The survey is part of a national project by the National Committee for Quality Assurance (NCQA), a non-profit group that helps people learn more about health care plans. Your answers will not have your name on them and will be part of a pool of information from others like you.

If you misplaced your survey letter, please call SPH Analytics at the toll-free number 1-888-797-3605. They will mail you another one. You can also call that number if you have any questions.
